1. Register for a PIN
   This PIN will act as your electronic signature for your online financial aid documents. Please request that the PIN be emailed to you.
2. Complete a 2008-2009 FAFSA using your 2007 federal tax information.
   You can fill this form out online and submit it with your PIN. Please remember to add Herzing College’s school code to your FAFSA form (School Code: 009621).

Stafford Entrance Counseling

To begin this session please choose “Wisconsin” under the state options and click on “Continue”.

1. Select “Herzing College Online Campus– Madison, WI” as your school and click on “Continue”.
2. Read through all directions carefully and proceed through the Entrance Counseling session.

This is a five step session. The first step involves reading through some very important information regarding Stafford Loans and answering relevant questions. As you progress through the session, you will be required to enter your personal information as well as provide contact information about personal references. This session will be very helpful in providing you with information about your loans and is required in order to complete the financial aid process.


3. After you have completed the session, a confirmation page will appear.
   Please print this confirmation page for your records.

Master Promissory Note

After you have completed the Stafford Entrance Counseling session above, please go to: https://dlenote.ed.gov/empn/index.jsp and complete a Master Promissory Note. Click on the “Complete New MPN for Student Loans” link at the bottom of the page. Read and click the “OK” button when the dialogue box appears. For all Undergraduate students (students enrolling in a Bachelor, Associate or Diploma Program) select the “Subsidized / Unsubsidized” option and click “Continue”.

In Step 3, you will be asked for your name and the school location. Please enter your name, select “WISCONSIN” then click “Continue”. On the next screen, please select “HERZING COLLEGE – 5218 EAST TERRACE DRIVE, MADISON, WI 53718426 – G09621” and click “Continue” again.

Additional Documentation

You may be asked to submit supplemental documentation once your file has been submitted to our Financial Services Department. Your FAFSA may be “Selected for Verification” by the Federal Government, which is a normal part of the financial aid process. If you are selected, you will be asked by Financial Aid to supply additional documents to your Financial Aid Officer, including a Verification Worksheet which will be emailed to you at that time. In order to complete the process in a timely manner, we highly recommend having the following documents ready for submission:

• 1040 Tax Form

A signed copy of your 2007 federal tax filings (and your spouse’s if applicable). If you are unable to locate a copy of these documents, you can request free Tax Transcripts from the IRS. Please call the IRS Tax Transcript Request Line at 1-800-829-1040.

• W2’s

Your 2007 W2’s (and your spouse’s if applicable).

• Social Security Benefit documentation

From 2007 for you and/or your spouse.
