Free Application for Federal Student Aid (FAFSA)

Go to: www.fafsa.ed.gov

1. Register for a PIN
   Your PIN will be used as your electronic signature for applications completed on the web. The PIN Site Link is located at the top of the page.
2. Complete the FAFSA
   (Currently completing 2011-12). You will need to have your PIN and corresponding federal taxes to complete the FAFSA on the web. Please remember to add Herzing University to your FAFSA. Our school code is 009621 Madison, WI.

Entrance Counseling

To begin please go to: https://studentloans.gov. Then click on the “Sign In”. Enter your personal information, click on “Sign In”. Then click on “Complete Entrance Counseling” then click on “Complete Entrance Counseling”.

1. 1. Step One: Counseling Type
• Indicate that you are “completing entrance counseling in order to receive loans as an undergraduate student”. Click “Continue”
2. Step Two: Guidelines
• Read over the information and click “Continue”.
3. Step Three: School Information
• To select Herzing University as your school:
1. Under School State select Wisconsin
2. Under School Name select Herzing University
• Click “Continue”
4. Step Four: Counseling Session/Quiz
• Review the provided information and complete all quiz questions and proceed through all the informational and quiz pages.
• To view your Rights and Responsibilities click on “You must view the Borrower’s Rights and Responsibilities for Direct Subsidized and Direct Unsubsidized Loans.”
• Click “Submit”
• Click on “Print” to print a copy of your confirmation page for your records

Direct Loan Master Promissory Note

After you have completed the Stafford Entrance Counseling session, you will need to complete the Master Promissory Note. The Master Promissory Note (MPN) is your agreement to pay back any student loans taken out on your behalf, should you decide to accept any as part of your Award Package.

Additional Documentation

You may be asked to submit supplemental documentation once your file has been submitted to our Educational Funding Department. Your FAFSA may be “Selected for Verification” by the Federal Government, which is a normal part of the financial aid process. If you are selected, you will be asked by Financial Aid to supply additional documents to your Educational Funding Advisor, including a Verification Worksheet which will be emailed to you at that time. In order to complete the process in a timely manner, we highly recommend having the following documents ready for submission:

• 1040 Tax Form

A signed copy of your most recent and/or previous year federal tax filings (and your spouse’s if applicable). If you are unable to locate a copy of these documents, you can request free Tax Transcripts from the IRS. Please call the IRS Tax Transcript Request Line at 1-800-829-1040.

• W2’s

Your most recent and/or previous year W2’s (and your spouse’s if applicable).
